Hi Dragon Hunter, ar present there are several trains between Xining and Lhasa. But for the peak seasonn of travel is coming, so the ticket may also not very easy to book. So you'd better book it in advance. Usually some travel agent can do it for you and so is the travel permit.
There is a 24 hour hotline on the top of this website. Maybe you can have a try to find the detailed infor..

Hi Drgon, the Tibet Permit costs nothing, but maybe the travel agent may charge you some fees for the help service.

No, Sisi, I have heard that the Tibet Entry Permit cost CNY 100, and the Alien's Permit CNY 50, usually service charges are implemented. Different travel agencies may offer different charges.
This is the charge I learn from Tibettrip.com, which is reletively lower.
