the bus runs from Lhasa Western Suburb Coach Station (Xi Jiao Ke Yun Zhan) to Xining at 10:00 and the bus fare is CNY 359 per person. the bus only runs twice a week. so you'd better consult the coach station in advance.

You have to apply for a nepal visa in person at the consulate of Nepal in Lhasa. The visa fee is US$ 25 for a 15-day multiple entry visa; US$ 40 for a 30-day multiple entry visa.

usually they are not included, but i'm not very sure, you may ask the conductor. the ticket is about CNY 520.
